Privacy Policy

Effective Date: September 25, 2026

Fede Family & Cosmetic Dentistry (“Fede Family Dentistry,” “we,” “us,” or “our”) owns and operates the website located at https://fedefamilydentistry.com (the “Site”). This Privacy Policy explains what information we collect through the Site, how we use it, and the choices you have. By using the Site, you agree to the practices described below.

If you do not agree with this Privacy Policy, please do not use the Site.

A note on health information: Any protected health information (“PHI”) you share with us as a patient — whether in our office, over the phone, or through a completed patient form — is governed by our HIPAA Notice of Privacy Practices, which is provided separately at our office and available upon request (see Section 11). This Privacy Policy addresses only the information we collect through the Site itself.

INFORMATION WE COLLECT

a) Information You Provide Directly. When you fill out a contact form or appointment-request form on the Site, we may collect information such as your name, email address, phone number, and any message or scheduling details you choose to share.

b) Online Appointment Scheduling. When you use our “Schedule Appointment” or “Book Today” feature, you are directed to MyDentalVisit, a third-party online scheduling platform. Information you enter there — such as your name, contact information, insurance details, and preferred appointment times — is submitted directly to MyDentalVisit and used to coordinate your appointment with our office. Please see Section 3 for more on this third-party service.

c) Patient Forms (Health and Treatment Information). Our Site makes certain patient forms available for download, including the Health History Form, New Patient Information Form, Consent for Use & Disclosure of Health Information, and the Request to Inspect or Copy Protected Health Information form. These forms are intended to be downloaded, completed, and returned to our office in person, by mail, or by fax — they are not submitted directly through the Site. Any health or treatment information you provide through these forms, or in the course of receiving dental care from us, is Protected Health Information under HIPAA and is handled in accordance with our HIPAA Notice of Privacy Practices, not this Privacy Policy.

d) Information Collected Automatically. When you visit the Site, certain information may be collected automatically, including your IP address, browser type, device type, referring/exit pages, and general usage data (such as pages viewed and time spent on the Site).

HOW WE USE YOUR INFORMATION

We collect and use information from the Site to:

  • Respond to inquiries and appointment requests submitted through our contact or scheduling forms;
  • Coordinate and manage appointments, including through our online scheduling provider;
  • Understand how visitors use and navigate the Site so we can maintain and improve it;
  • Communicate with you about an appointment, inquiry, or request you have made;
  • Maintain the security, functionality, and performance of the Site;
  • Comply with applicable legal obligations, including HIPAA where relevant.

We do not use information collected through this Site for advertising, marketing campaigns, or promotional email lists without your consent, and we do not sell or rent personal information to third parties.

THIRD-PARTY TOOLS AND SERVICES

Google Analytics

We use Google Analytics, a web analytics service provided by Google LLC (“Google”), to help us understand how visitors interact with the Site (e.g., pages visited, time on page, general geographic region, and referral source). Google Analytics uses cookies and similar technologies to collect and analyze this information in aggregate and pseudonymous form.

Google may use the data collected through Google Analytics in accordance with its own privacy policy. You can learn more at https://policies.google.com/privacy, and you may opt out of Google Analytics tracking by installing the Google Analytics Opt-out Browser Add-on (https://tools.google.com/dlpage/gaoptout).

PAYMENTS AND INSURANCE

We do not currently process payments through the Site. Payment for dental services is handled in our office and is accepted via major credit cards, CareCredit, check, and cash. If you choose to apply for CareCredit financing, that application is submitted directly to CareCredit (a service of Synchrony Bank), a third party, and is subject to CareCredit’s own privacy policy, available at https://www.carecredit.com.

COOKIES AND TRACKING TECHNOLOGIES

The Site may use cookies and similar tracking technologies (such as those used by Google Analytics, described above) to support Site functionality and analytics. Most web browsers allow you to control cookies through your browser settings, including blocking or deleting cookies. Please note that disabling cookies may affect certain features of the Site.

HOW WE SHARE INFORMATION

We do not sell, rent, or trade personal information collected through the Site. We may share information only in the following limited circumstances:

  • With service providers (such as Google Analytics and MyDentalVisit, described above) who help us operate the Site and manage appointments, strictly for the purposes described in this Policy;
  • If required to do so by law, subpoena, or other legal process;
  • To protect the rights, property, or safety of Fede Family Dentistry, our patients, or others;
  • In connection with a business transaction such as a merger, acquisition, or sale of assets, in which case information may be transferred as part of that transaction.

Sharing of Protected Health Information for treatment, payment, or healthcare operations purposes is governed separately by our HIPAA Notice of Privacy Practices.

DATA RETENTION

We retain information collected through the Site only as long as reasonably necessary to fulfill the purposes described in this Policy — such as responding to your inquiry or coordinating your appointment — or as required by law. Retention of patient health records is governed by our HIPAA Notice of Privacy Practices and applicable New Mexico recordkeeping requirements.

DATA SECURITY

We use reasonable administrative and technical safeguards designed to protect the information we collect through the Site. However, no method of transmission over the internet or electronic storage is completely secure, and we cannot guarantee absolute security.

YOUR CHOICES

You may choose not to provide certain information; however, this may limit your ability to use certain features of the Site, such as submitting a contact form or scheduling online. You may also disable cookies through your browser settings and opt out of Google Analytics tracking as described in Section 3.

CALIFORNIA CONSUMER PRIVACY ACT (CCPA)

If you are a California resident, the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA), provides you with certain rights regarding personal information collected through the Site. These rights include:

  • Right to Know — the categories and specific pieces of personal information we have collected, the sources, the purpose, and any third parties it was shared with.
  • Right to Delete — request deletion of personal information we have collected, subject to certain legal exceptions (including our recordkeeping obligations for patient records).
  • Right to Correct — request correction of inaccurate personal information.
  • Right to Opt Out of Sale or Sharing — we do not sell or share personal information collected through the Site for cross-context behavioral advertising, so no opt-out is necessary.
  • Right to Non-Discrimination — you will not receive discriminatory treatment for exercising your CCPA rights.

Categories of Personal Information Collected: Identifiers (name, email address, phone number) and internet or electronic network activity information (browsing behavior collected via Google Analytics), as described in Section 1.

To Exercise Your Rights: California residents may submit a request using the contact information in Section 12. We may need to verify your identity before fulfilling certain requests.

HIPAA NOTICE OF PRIVACY PRACTICES

As a dental care provider, Fede Family & Cosmetic Dentistry is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Our separate HIPAA Notice of Privacy Practices describes in detail how we may use and disclose your Protected Health Information, and your rights with respect to that information, including the right to request access to or a copy of your records using our Request to Inspect or Copy Protected Health Information form. Our Notice of Privacy Practices is provided to all patients and is available at our office upon request. This website Privacy Policy does not replace or modify our HIPAA Notice of Privacy Practices.

CHILDREN’S PRIVACY

The Site itself is not directed to children, and we do not knowingly collect personal information from children through the Site. As a family and pediatric dental practice, we do provide care to children, and any health information for a minor patient is provided to us by a parent or legal guardian and is handled under our HIPAA Notice of Privacy Practices, not this Site Privacy Policy.

CHANGES TO THIS PRIVACY POLICY

We may update this Privacy Policy from time to time to reflect changes in our practices or for legal or operational reasons. The updated version will be indicated by a revised “Effective Date” at the top of this page. We encourage you to review this Policy periodically.
